Does the cooling down also work completely at 12 V? So the box is switched off and warm, drinks are not cooled and I switch it on in the car and 30 minutes later I have ice-cold drinks or do I need 220V?

Yes, cooling down works completely with 12V. But you won't have ice-cold drinks in 30 minutes; a gastro cooler that is already pre-cooled will barely manage that.
If you fill the cooler completely with warm drinks, it will take a few hours until they are ice cold.
