Would I need an AVR with a second 'HDMI out' to connect my Sony MDR-HW700DS (unfortunately only with HDMI 1.4) headphones? I primarily watch 4K HDR movies from my NAS, but only have a 5.1 system.

You can first connect the peripherals via HDMI to the headphones, their out to the AVR and their out (ARC) to the TV (if ARC-capable).
