During normal operation, screws are lost while driving! 1. will these screws be replaced? 2. how is it possible that a tested product can have such defects?

Just finished a bike tour with all kinds of roads. The carrier didn't cause any problems, all the screws are still on. I had tightened all of them when I adjusted them.
