It's best to buy BOTH ; )
Depending on the type of shoes (sneakers<>trekking shoes<>moonboots) one or the other will fit.
PS Shoe sizes refer to your feet, this has nothing to do with the outer dimensions of the shoes...
I have shoe size 45/46 and the large fit well. That's why I would recommend small for 43.
