The microphone is good. But if you expect your iPhone to give good sound with it, the main question is which recording software you are using and whether it can work well with the microphone via the adapter. I advise you to always record sound with a suitable recording device. The internal iPhone microphones are usually better than a connected microphone with an adapter. Some apps may work better than others. Specifically Rode Rec Le is well tunable to this mic.
No there isn't, just the foam tube.
If there is a 3.5 mm jack socket, then yes. But I suspect that the D3100 does not have one. At least Nikon doesn't mention it.

You don't need an adapter for that. The cable from the mic should fit the camera if the mic socket is present and the slide on the mic should fit the flash mount on the camera. The only thing you need are the block batteries for the mic because they are not included ;-)
